A chemical assessment is a confidential, face-to-face interview. During this interview, you will be asked a variety of questions to determine:
Whether or not you meet diagnostic criteria for a chemical use disorder
The severity of chemical use problems
Insight into problems
Risk of future problems associated with substance use
Why do I need to complete a chemical assessment?
If you have encountered legal problems related to substance use (i.e. DWI, possession, underage consumption, failed interlock system) it is often recommended to be evaluated for a chemical assessment
Recommendation from your attorney prior to court appearance
Requested by your employer for use of alcohol or drugs at work
Your loved one(s) are concerned about your substance use
You are questioning if you need treatment and would like help with knowing which type of treatment would be best for you
I’ve never done a Rule 25 assessment before, what can I expect?
After scheduling an appointment, you will be sent a link to the client portal to complete the intake forms
The length of the appointment is 60 minutes
You will be asked to sign a Release of Information for two people that are close to you in order to verify information about your history
A written summary of the findings and recommendations will be completed within 2-3 business days
Your written summary and recommendations will be reviewed with you over the phone
The written summary can be sent to whomever you choose (with written permission).
How much does a chemical assessment cost?
The cost of a chemical health assessment is $250
